Responsibilities:

• Assess each patient and develop an appropriate treatment plan with measurable goals and time frame
  
• Prepare and submit clinical and progress summaries based on identified and attained goals
  
• Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
  
• Open/Supervise cases for LPTA's to do follow-ups
  

Qualifications:

• Active License of Physical Therapy in the state of Virginia
  
• Prior Homecare experience (preferred)
  
• EMR system: Kinnser Wellsky (preferred)
