Job Summary The Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.   Responsibilities Providing treatment as ordered by the attending physician, dentist, or podiatrist. Assisting in developing and updating the plan of treatment or Plan of Care (POC). Assisting the physician in evaluating level of function. Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record. Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program. Teaching and supervising other healthcare personnel when appropriate. Evaluating the home environment and making appropriate recommendations. Participating in in-service education programs.   Qualifications Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.  Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent. Licensed or registered to practice as a PT in the state of practice. Has the ability to read and follow written instructions and document care given. Is self-directing and has the ability to work with little direct supervision. Possesses and maintains current CPR certification. Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements. Must be able to verbally communicate in the language of the client and document all client-related activity practices.   This position is based on a pay per visit compensation model.
