Physical Therapist - PT PRN (Tubac/Rio Rico/Nogales)

PRN Physical Therapist

Assess, plan, organize, and provide rehabilitative services that improve mobility, relieve pain, increase strength, and prevent or correct disabling conditions resulting from illness or injury.

As a PRN Physical Therapist, you will:

• Perform and document initial evaluations, analyze clinical findings, identify patient needs, and establish a diagnosis before intervention.
• Develop and implement individualized treatment plans to maintain, improve, or restore physical function, alleviate pain, and prevent further dysfunction.
• Evaluate patients’ responses to treatment and adjust plans of care to achieve maximum benefit.
• Apply infection-control standards and universal precautions while providing patient care.
• Ensure continuity and quality of care through accurate, timely documentation.
• Communicate effectively with patients, families, team members, physicians, and other healthcare professionals.
• Promote and uphold the agency’s philosophy, policies, and procedures.
• Complete all required documentation within company-established guidelines.
• Accept patient assignments based on availability and organizational needs.

Requirements

• Successful completion of an accredited physical therapy program.
• Current, valid Physical Therapist license in the state of practice.
• Minimum of one year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and automobile insurance.
• Homecare Homebase experience preferred.
• Ability to work a flexible PRN schedule based on patient-care needs.

Physical Demands

• Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.
• Ability to lift, carry, pull, or push up to 100 pounds.
• Ability to travel to patients’ homes within the assigned service area.
