Post Rehab is seeking a Physical Therapist Assistant (PTA) to join our rehabilitation team. This role is a great opportunity for a motivated clinician who enjoys helping residents improve mobility, rebuild strength, and regain independence in a skilled nursing and post-acute care environment.

What You'll Do

• Provide physical therapy treatments under the direction of a licensed Physical Therapist
• Work with residents on strength, balance, gait, mobility, endurance, and functional activities
• Follow individualized treatment plans based on each resident's goals
• Monitor progress and communicate changes to the supervising Physical Therapist
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies
• Complete accurate and timely clinical documentation
• Collaborate with PT, OT, SLP, nursing, and other members of the interdisciplinary care team

Qualifications

• Active New York Physical Therapist Assistant (PTA) license required
• Strong communication and teamwork skills
• Skilled nursing or rehabilitation experience preferred but not required
• New graduates are encouraged to apply
