Your Role as a Physical Therapist:
You will play a key role in enhancing residents’ mobility, function, and quality of life. You’ll evaluate, develop, and implement treatment plans tailored to each individual’s goals—helping them regain independence and strength.
Key Responsibilities:

Perform initial and ongoing assessments of residents' physical conditions and functional abilities

Develop and carry out individualized treatment plans to reduce pain, restore mobility, and prevent further injury

Document all patient interactions, assessments, and progress in the Net Health EMR system (preferred)

Collaborate with caregivers, nursing staff, and interdisciplinary team members

Provide patient and caregiver education on treatment strategies and home programs
-
Monitor and adjust treatment based on patient response and progress

Ensure compliance with all federal, state, and facility policies and procedures
Qualifications:

Current PA license as a Physical Therapist in good standing (required)

Experience in SNF/long-term care settings preferred

Knowledge of current therapy best practices and protocols

Experience with Net Health EMR preferred

Ability to work with patients of varying conditions and personalities

Strong communication, organizational, and interpersonal skills
