PRN Physical Therapist (PT) – Home Health

As a Home Health Physical Therapist, you'll develop individualized treatment plans that improve mobility, reduce pain, restore function, and enhance quality of life.

What You'll Do

  • Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
  • Develop and implement therapeutic interventions to improve mobility, strength, balance, and functional independence.
  • Monitor patient progress, modify treatment plans as needed, and document outcomes.
  • Educate patients, families, and caregivers on exercises, safety, and disease management.
  • Collaborate with physicians, nurses, and the interdisciplinary care team to ensure coordinated patient care.
  • Maintain timely, accurate, and compliant clinical documentation.
  • Follow infection control practices and all agency policies and procedures.
  • Promote exceptional patient care while supporting the mission and values of the agency.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist (PT) license in the state of practice.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • One (1) year of home health experience preferred.
  • HomeCare HomeBase experience preferred.

Physical Requirements

  • Ability to stand, walk, bend, kneel, squat, crawl, and sit for extended periods.
  • Ability to lift, carry, push, and pull up to 100 pounds with or without reasonable accommodation.
