Job Title: PRN - Physical Therapist (PT) - Home Health

The Home Health Physical Therapist will be responsible for providing physical therapy services to patients in their homes. The Physical Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed. The Physical Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.

Responsibilities

• Conduct initial assessments of patients to determine their physical therapy needs.
• Develop treatment plans based on patient needs and goals.
• Provide physical therapy services to patients in their homes.
• Monitor patient progress and adjust treatment plans as needed.
• Communicate with other healthcare professionals to ensure comprehensive care for patients.
• Maintain accurate and up-to-date patient records.
• Provide education to patients and their families about their condition and treatment plan.

Requirements

• Current state license as a Physical Therapist
• Minimum of 1 year of experience in home health
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Excellent time management and organizational skills
• Valid driver's license and reliable transportation
