Job Description:

We are seeking a dedicated Physical Therapist for a Home Health team who will provide in-home services, focusing on improving patients' independence and overall functioning. This role includes patient assessment, program development, education, and progress reporting.

Key Responsibilities:

- Conduct assessments and create individualized treatment programs.

- Document and communicate patient reactions and any changes in condition.

- Guide patients and caregivers through therapy programs and the use of devices.

- Oversee the work of physical therapy assistants and certified nursing assistants as needed.

- Compile reports and engage in staff and organizational development activities.

Requirements:

- Master's degree in Physical Therapy.

- Must possess a valid Physical Therapist license in Illinois.

- Valid Driver's License and reliable transportation are essential.

- Open to new graduates or candidates with less experience in physical therapy.

Work Schedule:

- Monday to Friday, starting between 8:00 AM and 9:00 AM until 5:00 PM.

- One weekend per month and two holidays per year required.

- No on-call duties.

Travel Requirements:

- Local travel to patient homes within the Chicago area.

Compensation and Benefits:

- Pay-Per-Visit (PPV) remuneration structure, with details provided in attached documentation.

- $10,000 First Year Success Bonus.
