Schedule: PRN – Minimum 3 shifts per month

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health, you’ll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well-being.

As a Physical Therapist, you will:  

  • Provide Direct Client Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals. Perform re-evaluations as required to assess progress and adjust care plans accordingly. 
  • Maintain Accurate Client Records and Billing – Document all services accurately and promptly using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines. 
  • Develop and Support Follow-Up Programs – Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes. Make referrals to continuation programs as necessary to promote maximum independence and functionality.   
  • Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery. 
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.   

 This job also requires: 

  • Current Physical Therapist license in the state of Minnesota.
  • Strong collaboration and teamwork skills.
  • Clear and compassionate communication abilities.
  • Proficient in English for effective communication with staff, clients, and visitors. 
  • Successful completion of a state-mandated background check.
  • Able to physically perform the duties required by the position.
  • Must obtain CPR certification within 6 months of hire.
