Post Rehab is seeking a Physical Therapist to join our rehabilitation team in a skilled nursing setting. This role offers the opportunity to work with residents focused on improving mobility, strength, and overall functional independence.

Responsibilities:

• Perform physical therapy evaluations and establish individualized treatment plans
• Provide therapy focused on gait training, transfers, balance, strengthening, and mobility goals
• Collaborate with rehab and nursing staff regarding resident care plans
• Monitor progress and update treatment approaches as needed
• Maintain accurate and timely documentation

Qualifications:
