Responsibilities:<\/b><\/span>

• Responsible for the provision of therapy consultation, evaluation and services to patients.<\/span>
<\/span><\/span><\/div>
<\/span><\/span><\/div>
• Provide comprehensive physical therapy services to patients, ensuring their rehabilitation and well -being.<\/span>
<\/span><\/span><\/div>
<\/span><\/span><\/div>
• Maintain accurate, detailed reports and records.<\/span>
<\/span><\/span><\/div>
<\/span><\/span><\/div>
• Complete reports or assessments/evaluations and progress notes utilizing facility's approved documentation forms.<\/span>
<\/span><\/span><\/div>
<\/span><\/span><\/div>
• Perform care consistent with the scope of licensure and in accordance with the facility's policies and procedures.<\/span>
<\/span><\/span><\/div>

• Work effectively with fellow medical staff and treatment team members.<\/span>

• Comply with all state requirements and facility policies and procedures.<\/span>
