Position: Full-time or part-time Home Health Physical Therapist (PT)
Location: Thurston County. We are looking for passionate individuals dedicated to providing quality patient care in our new Home Health division.

Responsibilities

  • Provide comprehensive patient care through assessment, diagnosis, and treatment planning in home health settings.

  • Develop and implement individualized care plans, focusing on rehabilitation and mobility enhancement.

  • Educate patients and families on physical therapy techniques and home exercises.

  • Collaborate with interdisciplinary teams to ensure continuity of care and effective discharge planning.

  • Maintain accurate documentation of patient progress and treatment outcomes while adhering to medical terminology standards.

  • Stay current with best practices in physical therapy, particularly in hospice and geriatrics.

Qualifications:

  • Licensed Physical Therapist in Washington state (or before your first day of work)

  • One to two years of experience as a Physical Therapist

  • Current BLS certification
